KEEP YOUR HEALTH ON A HIGH NOTE

Musicians, do you know that you may qualify for the Family Health Plus program, a "free" N.Y. State sponsored health insurance program?

The Jazz Foundation of America has partnered with HealthFirst to try to help musicians who qualify, to get free health insurance from state of New York for you and your family.

You may qualify if:
Your income is $9,000 or less (single person) (For families, the income level is higher)
You live in one of the five boroughs of NYC or Long Island
You are between the age 19 and 64

To fill out an application, come to:

Birdland Jazz Club
315 West 44th Street
(between 8th and 9th Ave)

Monday, April 21, 2003 12:00 noon - 4:00 p.m.


To sign up you will need to bring a copy of:
-Social Security Card
-Proof of age (passport, driver's license, or state Photo ID)
-Proof of residency
-Four consecutive pay stubs or a signed statement stating your income is up
to $8,988 (single)

As a friend of mine (Buddy Deppendschmidt) has explained it to me many times "musicians and jazz musicians in particular don't have health insurance".
Really ... if I was still playing i couldn't afford it it is way out of reach.
In the old days when I was playing I carried major medical.

It kind of sucks.

BTW ... show me a person making $9000 a year and I will show you a skeleton.
